Obama's healthcare Joe the plumber moment

I am sure many of you caught Obama's response to the question pertaining to a 100 year old getting a pacemaker where he stumbled his way to finally saying that people/patients should consider taking a pain pill.

What is the meaning of this comment that seems out of context with the question asked? Like with Joe the plumber, in an unscripted moment, his true character and agenda slips out.....like being after socialization [[spreading the wealth) with Joe the plumber, he is after rationing care [[or withholding it) as part of his takeover of health care.
