Two very annoying News Headline phrases need to go ASAP:

"XXXXXX Car Jacking Gone Wrong..."

"XXXXXX Home Invasion Gone Wrong..."

'Gone wrong'? Are not Car Jackings and other criminal actions WRONG by definition, or only when some one is murdered, maimed or raped in the process?

What I really disliked was someone's secretary [[back when there were actual secretaries!) to call and tell me to "....please hold for Mr. $#%*&?%%#...."!

We may as well keep the word; the activity isn't going away, any time soon, so we need a word for it.

'gone missing', 'went missing'

What happened to disappeared or vanished?

'surgeries'

"The patient had four surgeries to repair the injury"

What happened to 'operations'? You don't have a surgery. You go into surgery where a Surgeon performs an operation.
