Would anyone know under what conditions a city engineer or inspector can go into a private home to see if it meets code and/or needs upgrading? For example, if there is mold in the home. What causes a city inspection?

Most inspections happen at the time of sale.

If your property is open to trespass then a officer from blight court can come in and write tickets based upon what he or she sees.

Residential rental properties are [[supposed to be) inspected annually. There's a fine for renting a property without registering it as a rental [[~$300)

it would take a warrant in 99% of the cases for an inspector to even trespass on your property, they can observe from the street and some municipalities even use sattelite imagery to determine additions/decks etc.....
if there is a question of safety, more than likely it was observed from the street or reported by another person and even then it would likely take a court order to have the city "inspect" the property...
at transfer/sale, the city will typically require an inspection.. although not all cities...
and when any work permits are pulled, although they are typically limited to inspect only the work that was proposed/completed....
